Incident Report
This is an Incident Report, not an insurance claim. An insurance claim, to the extent the injured rider is covered, is filed through the rider's bicycle club.
Accident reporting is a joint project of the bicycle clubs in The Villages. Its purpose is to capture injury related details to improve cycling safety in The Villages. Please complete this Incident Report form - in detail - for all crashes in Sumter, Lake or Marion counties, that occur on club rides, and when the cyclist is riding to and from a club ride.
Non-Members will initially see a contact information collection form. This information is needed so that any necessary follow-up inquiries can be accomplished. Once the Non-Member contact information is submitted, the Incident Report will be displayed.
After completing this Incident Report form, contact the injured cyclist's Club Ride Director to initiate a claim.
